New Students to the South Pasadena unified school district:   SPMS (2009-2010)
To register new students to SPUSD (Grades 6, 7 or 8) for the 2009-2010 school year, please contact the Middle School Registrar at (626) 441-5830, Ex. 3908. Registration is by appointment only. You must reside in South Pasadena and have three proofs of residency (see below) at the time of registration.  Registration must be completed by the parent or legal guardian of the student.  Parents with students at more than one school must register and show proof of residency at each site.

Registration Dates (2009-2010):

April 14 - July 24: Registration by appointment only.
 
July 25 - August 15: Summer Break. No registrations will be processed.

August 17- August 24: Registration by appointment only.

August 27- August 28: Registration by appointment only.


Required Documentation:

Items from each of the following four (4) areas must be presented at the time of registration. All documents must be original (no photocopies will be accepted), and must include the name of the parent/guardian and the South Pasadena address.


1. DRIVER'S LICENSE OR LEGAL PHOTO (I.D.) OF PARENT/GUARDIAN*


2. VERIFICATION OF STUDENT BIRTH DATE (Choose one [1] of the documents listed below):
  • Birth Certificate
  • Hospital Certificate
  • Student Passport

3. STUDENT IMMUNIZATION INFORMATION (Choose one [1] of the documents listed below):

  • California Immunization Record
    •   7th graders must be fully immunized for Hep. B, have 2 MMR vaccines & proof of Varicella vaccine.
    •   Out of state and out of country students must have proof of MANTOUX test for TB.
  • School Records
  • Doctor Records

4. THREE (3) FORMS (DOCUMENTS) OF RESIDENCY VERIFICATION (Choose one [1] document from each category):

  •  CATEGORY A: (Choose one [1] of the documents listed below):
    • Deed of Trust
    • Mortgage Payment
    • Receipt Rental Agreement
      • The document must be within the last year and must list ALL of the occupants names and the name, address and telephone number of the landlord
    • Property Tax Bill
    • Closing Escrow Papers
  • CATEGORY B**: (Choose one [1] of the documents listed below):
    • Gas Bill or connection receipt
    • Edison Bill or connection receipt
    • Water Bill or connection receipt
    • Time Warner Cable Bill or connection receipt
    • Telephone Bill or connection receipt
      • If submitting a connection receipt, an actual bill must follow it within one month.
  • CATEGORY C: (Choose one [1] of the documents listed below):
    • Current Car Registration
    • Car Insurance Statement
    • Health Insurance Statement
    • Current Bank Statement
    • Pay Stub
    • Current Federal or State Tax Forms
*For cases of guardianship, additional information may also be required.

**For a newly established residence verification from Category B, a signed, original receipt or statement on letterhead from the utility company will satisfy the requirement, provided the name of the parent/guardian and South Pasadena address were service will be established are listed.
